Sounds like an editable PDF is what you want. After the patient fills it out, the fill can be 'flattened' so nobody else can alter it. Try Xodo PDF editor. Drawboard always frustrated me.

I know that 12 volts will not charge my Book at all. But 12 volts will slowly charge the clipboard alone. It wasn't important enough to me to try 16 volts. I doubt there would be an issue, but it just wasn't worth the worry to me.
