    In Windows 8.1 it started out of the box. I thought the dock was bad so I exchanged it, the second one was the same way. With Windows 10 the dock was a lot better but lately it has started again. I don't know if it was the new firmware update.

    Maybe try uninstalling the driver in device manager (don't delete the driver). Then restart your computer and let it install the driver again and see if that helps.

    You may have a bad recover image. You can try downloading your Surface Pro 2 USB recovery from Microsoft again. Step 7 and 8 should have removed all partitions from the drive.

    I would first try with all enabled. On my Surface Pro I had to disable secure boot before I could boot from a USB. On my Surface Pro 3 I did not.
